Jeremy is a seasoned healthcare executive with extensive experience leading and growing complex healthcare organizations. He currently serves as President of Community eConsult Network (CeCN), the management services organization supporting ConferMED, a national eConsult and telehealth organization focused on expanding timely access to specialty care.

Throughout his career, Jeremy has held senior executive leadership roles across multiple sectors of healthcare, with experience spanning operations, strategic growth, physician and provider engagement, organizational development, and healthcare services expansion. He is passionate about building high-performing teams and developing innovative care delivery models that improve access, quality, and the patient experience.

At ConferMED, Jeremy is helping lead the organization’s continued national growth and its mission to connect primary care providers with specialty expertise, particularly for patients and communities facing barriers to specialty care.

Rachael Dobbs is Chief Product Officer at ConferMED and joined the team in 2021. She oversees the implementation and ongoing quality of ConferMED’s products along with the Specialty Network. Currently, ConferMED’s Specialty Network includes over 260 credentialed Specialists with over 1,100 state licensures. Rachael offers over 20 years of operations and administrative experience in providing direct primary care, specialty care and telehealth services.

Agi Erickson is the Chief Commercial Officer at ConferMED, where she leads business development, sales, marketing, and strategic growth for the organization’s innovative eConsult specialty care and clinical education services. She has been involved with ConferMED since its inception and joined the organization full-time in January 2018.

With more than 20 years of healthcare experience, Agi is passionate about advancing innovative, scalable solutions that improve access to specialty care and strengthen primary care. Her work focuses on building strategic partnerships and expanding the adoption of eConsults to help healthcare organizations reduce barriers to specialty care, support clinicians, and improve access to timely, patient-centered care.

Agi works closely with health centers, health systems, payers, IPAs, PCAs, HCCNs, and other stakeholders at the state and national levels to advance more accessible and connected models of care. Through these partnerships, she helps bring innovative specialty care solutions to primary care organizations and the communities they serve. ConferMED currently provides services across 37 states.

Maryann is Chief Strategy Officer at ConferMED and joined ConferMED in 2018. She oversees relationships with national and regional payers, develops payer strategies with both private and public insurers, is responsible for top-line revenue growth and creating opportunities for operational/technical innovation. She oversaw the project management of expanding eConsults across 6 states, to over 60 FQHCs as part of the Quest for Health Equity eConsult initiative. Maryann offers over 25 years of experience in the delivery of health care services in a variety of organizations.

Michelle Chapman is a strategic healthcare executive with more than 20 years of experience driving growth, innovation, and performance across complex healthcare ecosystems. She currently serves as Director of Client Strategy at ConferMED, where she is a member of the senior leadership team and a key advisor on national client strategy, clinical operations, and enterprise expansion.

In her role, Michelle leads strategic initiatives across a diverse portfolio of more than 200 healthcare organizations, including FQHCs, Medicaid programs, payers, and state-funded entities. She is instrumental in advancing FHQCs, optimizing specialty access through eConsult solutions, and aligning payer-provider partnerships to improve outcomes and efficiency. She has successfully led large-scale implementations, including grant-funded national programs, and developed data-driven strategies that enhance performance, reduce provider burden, and accelerate adoption of innovative care delivery models.

Michelle is recognized for her ability to bridge clinical, operational, and financial priorities—partnering closely with C-suite stakeholders to drive measurable results. She has built and led high-performing teams, implemented enterprise-level analytics and KPI frameworks, and consistently deliver significant growth in client utilization and engagement.

Prior to her current role, Michelle held leadership positions across major healthcare organizations, including UnitedHealthcare / OptumCare and ConnectiCare, where she specialized in provider network development, contract negotiation, and value-based care transformation. She began her career in pharmaceutical sales with Merck & Co., Wyeth Pharmaceuticals, and Stiefel, earning national recognition for performance and leadership.

Michelle holds a Bachelor of Arts and Sciences, Cum Laude, from Boston University. She also serves as a board member for the Women and Girls’ Fund, Main Street Community Foundation, where she supports initiatives focused on advancing equity and opportunity for women and girls in underserved communities.

Dan is a Family Physician who has been with CHC since 2003. He obtained his MD from University of Virginia Medical School in 1992 and completed the U.Conn Family Medicine Residency in 1995. In addition to providing direct patient care to his patients at CHC’s Meriden site, Dr. Wilensky serves in several roles in the Weitzman Institute focused on educating the next generation of health care providers and expanding their scope of practice. He is the Chief Preceptor to CHC’s APRN Residency Program, and provides the same support for remotely hosted residency programs. He also precepts in the Middlesex Family Medicine Residency, providing supervision to MD/DOs of 4 different training years. He likes yoga, movies, swimming, and spending time with family. He is an advocate for the benefits of a vegan diet, health care as a right not a privilege, and environmental issues.

Hayley is the Senior Implementation Manager at ConferMED and has been part of the team since 2019. She leads new client implementations and practice onboarding, ensuring teams are well-prepared for a seamless go-live and successful adoption. Hayley develops and maintains efficient workflows, supports day-to-day operations, and leads the referral coordination team.

Michael Malchiodi is ConferMED’s IT Business Systems Manager. As a member of the senior leadership team at ConferMED, Michael is responsible for all technology initiatives and financial remittance. His current focus is on updating and streamlining ConferMED’s technology stack while improving the overall security posture of the company. Michael holds a MS in Computer Information Systems from Marist College and a MS in Finance from Rensselaer Polytechnic Institute.

Maryann Souza is a Senior Sales & Marketing Manager at ConferMED, bringing over 20 years of experience in healthcare operations, program management, and business development. Throughout her career, she has led complex initiatives across health plans, physician groups, and nonprofit healthcare organizations, including launching telehealth services for 2.1 million health plan members and spearheading large-scale EHR implementations across dozens of medical offices. At ConferMED, Maryann manages a national sales territory, driving growth in specialty care access for primary care providers and health plans nationwide.

Maryann holds a Bachelor of Science in Health Science from California State University, Fresno, and earned her Google Project Management Certificate in 2025.

Outside of work, Maryann is a certified yoga instructor (YTT-200) and an avid birder. Recently relocated to the Portland area, she enjoys hiking local trails, tending her backyard bird sanctuary, and exploring her new home state.
